What to expect at your first visit:
 
When to Arrive:
For your initial appointment, please arrive 15-20 minutes early to complete all necessary paperwork.
We encourage you to download the new patient forms listed below and complete them beforehand to expedite the new patient registration process.

What to Bring:
Please bring your insurance card and your doctor's orders for physical therapy, if one was given to you.
Your paperwork (if you downloaded it beforehand)
Your calendar/ day planner to schedule follow-up appointments

What to Wear:
Please dress with loose, comfortable clothes appropriate for your injury (ie, shorts or loose pants for knee injuries) and comfortable shoes.
